
What to Expect
Welcome to ENT Center Of The Ozarks! Thank you for choosing us as your preferred ENT provider in Northwest Arkansas. Your first visit will involve a few special steps so that we can get to know you.
Being well-prepared for your appointment will ensure that the provider has all the needed information to deliver the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. All new patients should complete intake documents on the patient portal prior to their appointment. If you have any trouble accessing the patient portal, please call us at (479) 750-2080 and we will be happy to assist you. By completing and submitting the intake documents in advance, it will help ensure a quick and efficient check in process.
When to Arrive
We ask each new patient to arrive 30 minutes early for your initial appointment to ensure all necessary documents, insurance, and forms are completed prior to being seen by one of our providers. If you have completed your forms via the patient portal prior to your first visit, please arrive 10 minutes early. A legal guardian must accompany any minor to all appointments.
Checking in
When you first arrive at our offices for your visit, please check in at the front desk and bring the following items with you:
- Health insurance card of patient
- Driver’s License or photo identification of the responsible party (self or parent/guardian)
- List of current medications (if it was not completed via the patient portal in advance)
- Any prior imaging (CT scan) or medical notes from the referring provider, if applicable
Co-Pays and Other Payments
ENT Center Of The Ozarks accepts most medical insurances including Aetna, Blue Cross Blue Shield, Cigna, Humana, Medicaid, Medicare, United Healthcare, and many others. If there is a specific insurance you are interested in, please call your insurance company to check network status or our office at (479) 750-2080 and we will be happy to connect you with our billing department.
Your co-payment and deductible are required at the time of service. Upon check-in, our receptionist will review the patient’s insurance benefits with the patient. While our team works diligently to secure approvals from your insurance company, there may be instances where certain necessary and recommended procedures are not covered under your co-pay or may not be covered at all. If you have any concerns about potential costs and would like an estimate, please ask.
We accept Visa, MasterCard, American Express, Care Credit, debit cards, cash, and checks.
For new self-pay patients, a deposit of $200 is expected upon check in on the day of your appointment before treatment from your provider. The balance is due upon completion of visit.
Surgery payments are due in full one week prior to surgery. We accept payments via the patient portal, telephone, or in person.
Referrals
If you have been referred to us from another provider, we look forward to assisting in your care. Please allow 2 to 3 business days for referrals to be processed by our office. Once your referral has been processed, our receptionist will call you to schedule your appointment.
Late and No Show Policy
Patients who arrive after their scheduled appointment time may be rescheduled. If an appointment is not kept and has not been cancelled at least 24 hours prior to the appointment time, a $50 fee may be charged to the patient’s account.
