Our office is located on the 2nd floor of the Jones Clinic (part of Northwest Medical Center) just east of Hwy 71B. Exit Hwy 71B on to West Maple Ave. (just north of Hwy 412 and south of Emma Ave.). A free parking deck and lot will be on your left.

Bentonville Office:
Northwest Medical Plaza, 2900 Medical Center Parkway, Suite 140, Bentonville, AR, 72712.

Our office is located just west of I-540 in the Northwest Medical Plaza (connected to the Northwest Medical Center Hospital). From I-540, take exit 85 and turn west on South Walnut Blvd. Go ¼ mile and turn right on Medical Center Parkway. Free parking is located in front of the Medical Plaza.
